Acceptance of Terms

By enrolling in the FIRST Membership, the member agrees to these Terms & Conditions.

1. Membership Fee and Duration
   • The membership fee of $250 USD is charged annually and is non‑refundable.
   • Membership is valid for one (1) year from the activation date.
2. Benefits
   Members are entitled to the following benefits during the active membership period:
   • Fifty (50) free shipments to U.S. addresses.
   • Free shipping for returns and RMA items.
   • Free laser marking and free dot marking on eligible lenses.
   • Unlimited prescription revisions for the same patient within 60 days of the original order.
   • Two complimentary scleral lenses after reaching a cumulative spend of $2,000 within the membership period.
3. Exclusions & Limitations
   • Benefits apply only to the purchasing member account.
   • Benefits cannot be transferred, shared, or redeemed across multiple practices or customer accounts.
4. Reward Redemption
   • Complimentary scleral lenses must be redeemed within the same membership period in which the spending threshold was met.
5. Modification of Terms
   • ClerVision reserves the right to modify or discontinue the FIRST Membership program—including pricing and benefit structure—at any time with reasonable notice.
6. Termination
   • ClerVision may suspend or terminate membership for misuse, fraudulent activity, or violation of these Terms & Conditions.
   • No refunds will be issued in such cases.
7. Governing Law
   • These Terms & Conditions shall be governed by the laws of the United States and the state where ClerVision lenses are prescribed and shipped to.
